Can an old masonry fireplace take a gas insert?

Usually yes, but the flue decides, not the opening. Pre-1980 ranch and bungalow stacks here have clay tile liners sized for wood smoke. A sealed direct-vent insert needs its own vent path inside that chimney. That means a liner run top to bottom, terminated and sealed at the crown. Deteriorated flue tile mortar joints, a cracked crown, or seismic cracking left over from Sylmar and Northridge all have to be dealt with first. You cannot anchor new vent to crumbling brick.
